This is simply a small step up from a Genin Team. It normally consists of three ninja who were formerly a part of the same Genin Team, and a leader. The leader may, but need not be replaced, and members who have gone MIA may also be replaced. In some cases, usually with the permission of a Kage, a member may be higher or lower rank than a Chuunin.

The only addition to this from a Genin team, is a purpose. Aside from accomplishing missions, Chuunin squads also have special purposes, whether they are constant like spying on other hidden villages, or conditional such as them being an assassination squad during times of war.
